 sounds like a real bad ear mite infection . you need to TOTALLY clean all that out of it's ear and treat for mites . some anti-biotics and steroid drops wouldn't hurt as well  . did you take the dog to the vet and have it looked at and or eliminate a yeast or mite infection or was it done via a phone conversation ?
